1ST SEPTEMBER 2017

We are limiting the postcode area for acceptance on to our

list to G64 only

We apologise for any inconvenience. We have patients on the list with G33 and G21 postcodes. At the moment we are not actively removing these postcodes but if you move house within the G33 or G21 postcode you will require to find an alternative GP. As ever we try to offer the best service to our patients and owing to the ever increasing numbers the partners have decided that we need to limit the postcode area we accept patients from.

THE PARTNERS OF SPRINGFIELD MEDICAL PRACTICE

New Patient Registrationregister_now

Patients living within the G64 postcode area are welcome to register. In order to do so you should bring your NHS medical card and either a passport or photographic ID.  A member of the reception team will ask you to fill in the necessary forms.

If you prefer a particular gender of doctor we will note this and do our best to respect your choice. However, not all the doctors in the practice provide all services and specific doctors may not be immediately available.

Please contact reception for further information.

Changes to Personal Details

If you change any personal details at any stage after registering with the practice i.e. name, address, telephone number, it would assist us greatly if you let know.  Please speak to one of our receptionists who will ask you to complete a form so we can update your medical records.

Guide to GP Services

The Royal College of General Practitioners has produced a useful guide for patients about the services on offer at GP Surgeries and how to access them.  You can download the guide below.
